I work for MedQuist and we use the DocQscribe format. We download voice files in the system and then upload reports when completed. Not all transcriptionists type all types of reports, some handle strictly radiology, some strictly clinic reports, and others type all acute care reports (hospital). The wider range of the reports you can type can have a big difference in the amount per line you make. As radiology are considered some of the harder type reports, they give you a line differential when you can type these.

typically yes, otherwise sometimes physicians can pick and chose transcriptionists and vice versa, good luck

They should all be getting a variety of the different reports and physicians available. If you can’t or won’t transcribe all types of reports you should find another job. Transciptionists get paid to transcribe what is given to them. It’s not pick and choose!
